She works with women moving through perimenopause and menopause, and she also sees men in midlife and couples. Care is talk therapy only, and the FAQ says the practice does not prescribe medication, though she will discuss a referral to a prescribing physician and can speak with your own doctor if you give written permission. For menopause she uses cognitive behavioral therapy for the distress that comes with hot flashes and CBT for insomnia for sleep trouble, alongside acceptance and commitment therapy and emotionally focused therapy. Sessions are 50 minutes, usually weekly at first and then spaced further apart, and she says most clients need five to fifteen of them. She is a lifelong competitive athlete who says she went through her own midlife upheaval, which is what led her to start the practice.

Marnatti sees women at UK HealthCare's Women's Health Primary Care clinic in Lexington, and she is one of two clinicians who staff the health system's Midlife Health and Menopause Clinic. She joined the women's health practice in 2020 and moved into the menopause clinic when it opened in February 2025. That clinic runs Tuesday mornings as a consult service, and it pulls in UK specialists in bone health, nutrition and integrative medicine when a woman needs them. Visits are booked through the women's health clinic, and she also keeps a general primary care panel, so the same woman can bring her blood pressure or her bone density to the visit. On hormones she is plain about the evidence. After the FDA removed its black box warning she went on the health system's podcast to say the old label frightened women and prescribers away from treatment, and that she still weighs age, time since the last period and health history before starting anything.

What she says she enjoys most in midlife care is walking a woman through the pros and cons of hormone therapy so the choice is her own. The practice is openly cautious about compounded bioidentical hormones and makes the point that plenty of prescription bioidenticals are not compounded at all. Hormones are prescribed as pills, patches, gels, sprays and vaginal products, and bone density screening and blood draws happen in the office.
